- Local Services Tax Exemption Certificate - must be completed by an employee before we are permitted to make this $52 tax exempt. Required documentation must also be included.
- Local Services Tax Refund - If this tax is withheld and later determined that it shouldn't have been, a refund must be requested through the Capital Tax Collection Bureau.

- - Non-exempt staff may complete this online fillable form to submit donated vacation day(s) to Human Resource Services to participate in the Sick Leave Bank. (Please note: Your network username and password may be required to access and sumbit this online form.)
- Tuition Assistance Form - to apply for assistance for your child to attend an institution other than Dickinson
- - to apply to a tuition exchange school
- Tuition Remission Form - to apply for remission for tuition at Dickinson
- Workers Compensation - If you are injured on the job, you should notify your supervisor or department coordinator and report your incident by logging into the Dickinson and clicking on the WorkComp icon to complete the automated online form. If you experience difficulties with the online form that cannot be remedied after contacting Human Resource Services at ext. 1503 or hrservices@dickinson.edu, then complete a paper Incident Report Form. Treatment Authorization Forms for either Concentra Urgent Care or UPMC Urgent Care should also be completed prior to seeking treatment for an injury at work. Supervisors are requested to complete an Accident Investigation Form regarding their subordinates who experience a work injury.
